What does a Koh Tao clinic treat day to day ?

The bulk of the work is unglamorous and highly repetitive, which is precisely why it is done well. Island medicine is defined by its caseload rather than its equipment, and on Koh Tao that caseload is remarkably consistent from week to week. A doctor here will see the same dozen presentations hundreds of times a year, which builds a pattern-recognition that a generalist elsewhere simply does not get the volume to develop.

Injuries

Which tests are available on the island ?

Laboratory testing is available at the Koh Tao clinic, covering blood counts, organ function panels and testing for infections including dengue, malaria and influenza. Turnaround varies by test — the doctor will tell you at the consultation which results come back the same visit and which take longer, so nothing about your treatment plan rests on a result that has not arrived yet.

Imaging is where geography starts to matter. Digital X-ray is available at our Koh Phangan clinic rather than on Koh Tao. For a suspected fracture that means assessment, immobilisation and splinting here, with imaging on the next island along. CT scanning is not available at any of our clinics and is arranged by referral.

Where is the line between clinic and hospital care ?

The line falls at anything requiring an operating theatre, an inpatient bed, intensive care, advanced imaging, or a specialist opinion. Koh Tao has no hospital of its own, so you will be referred off the island for orthopaedic fixation, appendicitis, significant head injuries, serious burns, complicated pregnancy and cardiac events. That referral is not a failure of the clinic; it is the correct clinical decision, and making it early is the whole point.

What "stabilised and referred" actually means

It means the clinic does the urgent work before you travel rather than sending you away to have it done. A patient who arrives at a receiving hospital already assessed, stabilised and carrying notes gets treated faster than one who arrives cold, and on a route that involves a ferry that difference is measured in hours rather than minutes. In practice the sequence runs:

1. Assess and diagnose as far as the available equipment allows.
2. Control the immediate problem — bleeding, pain, airway, fluids.
3. Immobilise fractures and suspected fractures for the journey.
4. Document the findings, the treatment given and the working diagnosis.
5. Notify the receiving facility and your insurer before you travel.
6. Arrange transport appropriate to how urgent the case is.

The patient's job in all of this is short: hand over an accurate history at the start, and keep the paperwork you are given.

Why is diving the exception on this island ?

Because Koh Tao is one of the busiest dive-training destinations in the world, and dive medicine is the one field where a clinic here is likely to have more relevant experience than a large general hospital elsewhere. Ear barotrauma, sinus squeeze, marine envenomation and decompression illness are routine presentations rather than rarities. We are partnered with the SSS Recompression Chamber Network, and hyperbaric oxygen therapy is available for decompression sickness and arterial gas embolism — the chamber is on Koh Tao itself, so treatment does not depend on getting off the island first.

The important behavioural point is this: report post-dive symptoms immediately, however trivial they feel. Decompression illness is treatable, and outcomes depend heavily on how quickly treatment begins. Waiting overnight to see whether the tingling resolves is the one gamble on this island that is genuinely not worth taking. How does treatment continue if you island-hop ?

It continues without restarting, because the same group runs all three clinics. Stitches put in on Koh Tao can be removed in Lamai. A course of dressings begun here can carry on at Koh Phangan. Results and notes stay within one system rather than being reconstructed from your description at each new clinic.

The practical instruction is simply to say where you are going. Tell the doctor your itinerary at the first visit and the follow-up gets planned around your ferry tickets — including, where relevant, sending you to Koh Phangan for the X-ray you were always going to need anyway.
